  A 48 YEAR OLD MALE CAME WITH PAIN ABDOMEN AND LOW BACK PAIN Hi, I am Anamika Maiti, 3rd sem medical student.This is an online elog book to discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio. CASE SHEET: Chief complaints: A 48 year old male, industrial worker by occupation came to OPD with chief complaints of: -pain abdomen since 5 years -constipation since 5 years -pain in neck region since 1 year -low back pain since 1 year -shortness of breath since 1 year HISTORY OF PRESENT ILLNESS: The patient was apparently asymptomatic 5 years back. He then developed pain abdomen which was left side and umbilical region each episode lasted for 4 to 5 hours.  He also had complaint of constipation since 5 years.No history of blood in stools. A 60 YEAR OLD MALE WITH LEFT LOWER LIMB MONOPLEGIA SECONDARY TO ACUTE ISCHAEMIC STROKE. A 60 year old male who is an agriculture labourer  by occupation a resident of Nalgonda, came to the OPD with complaints of weakness in left lower limb since 20 days. HISTORY OF PRESENT ILLNESS: The patient has complaints of weakness in left lower limb since 20 days. The patient was apparently asymptomatic 2 months ago. A 40 year old male with pleural effusion A 40 year old male patient painter by profession,came to the OPD with the Chief complaints=  Shortness of breath for 3 days, appetite loss, weight loss since 10 days.He is complaining of chest pain since 4 days.The patient was admitted 2 days ago.  History of presenting illness =     patient was asymptomatic 10 days back, lost appetite and weight, developed shortness of breath 3 days back. Patient was unable to sleep at night due to SOB. Patient cannot breathe properly without oxygen mask.No history of fever, vomiting, coughing, wheezing.
